The Admission Process

Thank you for considering Madonna Heritage Assisted Living. Our staff is here to help and will do everything we can to make your loved one’s move as easy and comfortable as possible. Listed below are key steps in the admission process. If you have any question or need assistance during this process, please let our admission team know.

A Visit to Madonna Heritage

We strongly recommend that everyone involved in the move of you or your loved one to Madonna Heritage visit our community beforehand. Visits during regular business hours are best, so you will have the opportunity to meet the staff. If your schedule permits, we would be happy to have you join us for lunch. Please give us notice a day ahead of time so that our dining room staff can make the necessary arrangements.

Application Form

The form in this packet along with your deposit and a one-time assessment fee constitutes a complete Resident Application. State regulations require that this form be filled out completely.

Physical Exam

All residents must have a physical exam within 30 days prior to admission. The exam must be performed by a physician and must include a tuberculosis (TB) test or a chest x-ray and completion of the State Physician Assessment Form. The completed form must be submitted to Madonna Heritage within 48 hours prior to admission.



Health and Functional Assessment

The purpose of the state required assessment is to ensure that Madonna Heritage can meet the medical and social needs of each of our residents. To arrange an appointment, just give our admissions team a call. They will be happy to make the necessary arrangements.

The Lease

An Admissions Agreement outlining services offered at Madonna Heritage completes the admission process. The term of the Agreement is monthly but can be terminated under certain conditions with five days written notice to the Director of Admissions.

Required Information

Madonna Heritage will need to have copies of the following documentation on file. Please provide these documents and our staff will make the necessary copies and return the originals to you.

  • Power of Attorney Funeral arrangements

  • Do Not Resuscitate (DNR) Order

  • Private Insurance Cards

  • Living Will Guardianship Papers

  • Medicare Cards
